Hospice care focuses on the care, comfort, and quality of life of a person with a serious illness who is approaching the end of life. Hospice is designed for a situation where it may not be possible to cure a serious illness, or a patient may choose not to undergo certain treatments.

Grace Resolutions Home Health Provides:
- Hospice physicians collaborate with each person’s regular physician.
- Registered nurse case managers care for patients during routine visits and manage the patient’s individualized plan of care.
- On-call nursing is available 24 hours a day, 7 days a week.
- Hospice care aides conduct scheduled visits and assist with personal care.
- Medical social workers provide assistance with resources, emotional support, and a variety of other needs.
- Chaplains provide spiritual counseling and support.
- Medications, supplies, and equipment related to the terminal illness are provided.
- Respite care is available as a relief for caregiver exhaustion. Short-term respite stays occur in the long-term care facility setting.
- · Bereavement services support the family after the loved one has passed.
- Trained volunteers are also available for additional companionship and support.
- Special services related to comfort care are available if appropriate from a physical therapist, occupational therapist, speech therapist, and a dietary counselor.
