In-Home Medicaid Care (HCBS-Home and community based services)
Home and community-based services (HCBS) provide opportunities for people enrolled in Medicaid to receive care in the home as directed by the agency. The federal government program provides services to the elderly, people with disabilities, and clients with mental illness who need Medicaid in-home care in a familiar setting.
Medicaid in-home health care services often enable a person to remain at home rather than move to a skilled nursing facility.
Some of our services include:
- Personal care (grooming, bathing, and dressing).
- Assistance in transferring.
- Advanced personal care.
- Light housekeeping and laundry.
- Grocery shopping and errands.
- Preparing meals and cleaning up.
- Range of motion.
- Maintenance nursing (medication set-ups and assessment).
- Respite and advanced respite care for family caregivers.
In-Home Nurse Visits
Our skilled home health services give us the opportunity to provide skilled, professional health services to people who meet the criteria for Medicare home health coverage from a licensed medical professional. You or a loved one must meet these requirements to receive skilled home health:
1. The person requires intermittent skilled health care services provided by a nurse, physical therapist, speech therapist, or occupational therapist
2. The person is home-bound, meaning they have an illness, medical condition, or injury that restricts their ability to leave home without the assistance of others, the aid of supportive devices (such as crutches, canes, wheelchairs, or walkers), the use of special transportation, or those instances where leaving the home is medically contraindicated.
